How to apply for Medicare Savings Program (Arizona) — QMB / SLMB / QI-1

Arizona Health Care Cost Containment System (AHCCCS)

Who it's for

Medicare Savings Programs help people with Medicare pay their Part B premium, and (at the QMB level) also deductibles, coinsurance, and copays. Arizona has three levels — QMB, SLMB, and QI-1 — with rising income limits. In 2026 the highest level (QI-1) reaches about $1,796/month for one person and $2,435/month for a couple. Arizona applies NO asset/resource test to these programs. Apply at Health-e-Arizona Plus.

The income limit shown is the highest level (QI-1, 135% FPL). QMB (100% FPL, full premium + cost-sharing help) and SLMB (120% FPL) cut off at lower income. Arizona applies NO resource/asset test to the Medicare Savings Programs (the AHCCCS chart shows N/A) — do not assume the federal asset limits apply here. QMB enrollees should not be billed for Medicare-covered services.
