How to apply for Medicare Savings Programs (QMB, SLMB, QI-1)

Who it's for

Medicare Savings Programs help people with Medicare pay their Medicare costs. QMB pays Medicare premiums and the 20 percent coinsurance, and comes with a Medicaid card. SLMB and QI-1 pay the Medicare Part B premium. Alabama's monthly income limits (effective February 2026) are QMB $1,350 / $1,824 (one person / couple), SLMB $1,616 / $2,184, and QI-1 $1,816 / $2,455. In Alabama there is no resource (asset) limit for these programs. You must live in Alabama and be eligible for Medicare Part A. Mail or email the application (Form 211) to Alabama Medicaid, or call the Recipient Call Center.

If only one spouse has Medicare, that spouse's income must be under the individual limit and the couple's combined income under the couple limit. QI-1 funding is limited. Alabama Medicaid makes the final decision.
