How to apply for Illinois Medicare Savings Program (QMB / SLMB / QI)

Illinois Department of Healthcare and Family Services (HFS)

Who it's for

Illinois Medicare Savings Programs help Medicare beneficiaries pay Medicare costs. QMB (income ≤100% FPL) pays Part A & B premiums, deductibles, and coinsurance; SLMB (≤120% FPL) and QI (≤135% FPL) pay the Part B premium (~$185/month in 2026). DISTINCTIVE: Illinois has NO asset test for MSP — only income counts. QMB/SLMB/QI enrollees also get Extra Help (Part D low-income subsidy) automatically. Apply through ABE (abe.illinois.gov) or with SHIP counseling.

Illinois MSP has NO asset test (income-only) — more accessible than most states. QI (≤135% FPL) alone saves ~$2,220/yr in Part B premiums + automatic Extra Help.
