How to apply for Hart-Supported Living Program

Who it's for

The Hart-Supported Living Program makes grants for flexible, individualized supports that help Kentuckians with a disability live in their own homes and communities and avoid institutional care. Funds can go toward a broad, person-centered range of supports chosen to promote community participation, independence, and skill-building. There is no income test. It is open to a Kentuckian of any age with a disability as defined by the Americans with Disabilities Act. Because grant funding is limited, applications go through an individualized review-and-funding process — meeting the disability criterion does not guarantee a grant.

Hart-Supported Living makes individualized grants, and funding is limited — having a qualifying disability does not guarantee a grant. Applications go through a review-and-funding process; contact your regional coordinator (PCAP.HSL@ky.gov) to apply.

What you'll need

  • Proof of Kentucky residency
  • Documentation of a disability as defined by the ADA / KRS 210.770
  • Hart-Supported Living application (HSL New Application), submitted to the regional coordinator
