How to apply for Managed Long-Term Care (MLTC)

Who it's for

Managed Long-Term Care (MLTC) is how New York Medicaid delivers home- and community-based long-term care so older adults and people with disabilities can stay home instead of entering a nursing home. An MLTC plan coordinates home health aides, personal care, adult day health, nursing, therapies, and Consumer Directed Personal Assistance (where you hire your own aide). To enroll you must have Medicaid, be assessed (through the NY Independent Assessor) as needing a nursing-home level of care for more than 120 days, and be able to stay safely at home with the plan's help. MLTC includes partial-capitation plans, Medicaid Advantage Plus, and PACE.

MLTC requires New York Medicaid plus an Independent Assessor finding of a nursing-home level of care needed for more than 120 days, confirmed by individual assessment, so this shows as a may-qualify result. Enrollment help is available through New York Medicaid Choice (1-866-712-7197).
