How to apply for Medicare Savings Programs in California (QMB, SLMB, QI)

California Department of Health Care Services (DHCS), through county Medi-Cal offices

Who it's for

California's Medicare Savings Programs are part of Medi-Cal and pay some or all of your Medicare costs. Qualified Medicare Beneficiary (QMB) pays Medicare Part A and Part B premiums, deductibles, and co-insurance, for income up to 100% of the federal poverty level. Specified Low-Income Medicare Beneficiary (SLMB) pays the Part B premium, for income up to 120%. Qualified Individual (QI) pays the Part B premium, for income up to 135%. For 2026 that is about $1,816 a month for one person or $2,455 for a couple at the QI level, after the $20 that is not counted. You must have or be eligible for Medicare Part A. Apply at your county Medi-Cal office with the Medicare Savings Programs application (form MC 14A), or ask a HICAP counselor at 1-800-434-0222 for free help applying.

Shown against the highest tier (QI); lower income qualifies for more help (QMB also pays Medicare deductibles and co-insurance). The dollar figures are the 2026 federal poverty-level standard that California applies and change each year. You must have or be able to get Medicare Part A. California also checks assets, with a limit far higher than other states (DHCS lists $130,000 for one person from January 2026). People who get other Medi-Cal cannot get QI. Your county Medi-Cal office makes the final decision.
