How to apply for Social Work In-Home Support Program (Connecticut DSS)
Connecticut Department of Social Services (DSS), Social Work Services
Who it's for
The Social Work In-Home Support Program helps Connecticut adults with physical or mental disabilities stay in their homes instead of moving to a nursing home or other institution. As part of a DSS case plan, it can provide non-medical home care: adult day care, a companion, case management, chore help, home-delivered meals, homemaker services and a personal emergency response system. To qualify, income must be at Medicaid-related levels, assets at SSI-related levels, a doctor or other health professional must certify the need, and a DSS social worker must assess you. Apply with the Social Work In-Home Supports request form on the DSS page.
Only for adults whose disability means they would otherwise need institutional care. Income must be within Medicaid-related limits and assets within SSI-related limits; DSS does a social work assessment and decides. Older adults may be pointed to the Connecticut Home Care Program for Elders instead.
