How to apply for Lee Health Financial Assistance (Lee Memorial Health System)
Lee Memorial Health System d/b/a Lee Health — a public health system established by special act of the Florida Legislature in 1968, governed by an elected Board of Directors
Who it's for
Lee Health (legal name: Lee Memorial Health System) is one of the largest public health systems in Florida and the third TRUE public hospital district in this catalog — joining Sarasota Memorial and Halifax Health. Established by special act of the Florida Legislature in 1968 and governed by an elected Board of Directors, Lee Health operates five hospitals across Lee County: Lee Memorial Hospital, HealthPark Medical Center, Cape Coral Hospital, Gulf Coast Medical Center, and Golisano Children's Hospital. Lee Health's Financial Assistance Policy is one of the most generous in the catalog: patients with gross family income at or below 200 percent of the Federal Poverty Level usually pay NOTHING for qualifying hospital and physician services (100 percent discount). Patients with family income above 200 percent FPL but at or below 400 percent FPL pay only 20 percent of qualifying charges (80 percent discount). Even at higher income, patients qualify for full charity when total hospital and physician charges exceed 25 percent of household income — a catastrophic-cost provision that protects middle-income families from medical bankruptcy. Apply by completing the Financial Assistance Application (FAA) with income documentation; financial counselors at Lee Health can help complete the form. Mail completed forms to Lee Health Patient Financial Services, PO Box 150107, Suite 203, Cape Coral, FL 33915 or call (239) 424-1500 or 1-800-809-9906.
Lee Health Financial Assistance has tiered benefit levels: 200% FPL or below = 100% discount (you pay nothing); 200-400% FPL = 80% discount (you pay 20%); above 400% FPL = 100% discount when total bills exceed 25% of household income (catastrophic provision). The modeled $62,400 cap is 400% FPL for a single-person household 2026 — the OUTER bound for sliding-scale assistance; the FULL 100% benefit starts at $31,200 (200% FPL). Each tier is determined per encounter and requires the Financial Assistance Application with income documentation. You must also be denied for or have applied to other coverage (Medicare, Medicaid, Marketplace) before Lee Health FAP is approved. The catastrophic provision (above 400% FPL) is particularly valuable for middle-income families facing large hospital bills.
