How to apply for Citrus Connection PT Connect ADA Paratransit (Polk County)

Who it's for

PT Connect is Citrus Connection's ADA-mandated paratransit service for Polk County residents whose physical or cognitive disability prevents independent use of the fixed-route bus system. The service is door-to-door, shared-ride, and operates throughout Polk County in accessible vehicles. Eligibility is functional, not categorical — disability alone does not guarantee eligibility. Apply by downloading the ADA Application Short Form at ridecitrus.com, completing the applicant sections, and having an approved health care professional complete and sign the medical verification sections. The signed application is returned to Citrus Connection for review; the determination of eligibility is made within 21 days of receipt of a completed application.
