Who it's for
Integrated Care at Home brings medical care to you instead of you travelling to it. A nurse practitioner provides primary care in your home. A licensed counsellor can help with anxiety, depression, phobias or addiction. A community health worker helps you coordinate home and community services and work towards your health goals. A registered dietitian gives practical nutrition advice. The team is backed by social workers and nurses at the Area Agency on Aging. It is designed to cut emergency room visits and hospital stays.
You need to be 18 or over, mostly homebound or unable to get out to appointments easily, and managing a long-term condition such as diabetes, chronic kidney disease or heart disease. This is NOT free care: Integrated Care at Home bills your insurance and accepts most plans including Medicare and Medicaid, so check what your plan covers.
