How to apply for Supplemental Nursing Care (SNC)

Who it's for

SNC is monthly cash help for older, blind and disabled Missourians who live in an assisted living or residential care facility and cannot cover the fees. It pays you a $50 monthly personal needs allowance, plus a cash payment toward your facility fees — $156 a month for residential care, or $292 a month for assisted living. The Family Support Division compares your monthly income from work, Social Security and any other source against what your facility costs; if your income is less than the facility cost, SNC can help cover the difference up to those limits.

Where to apply

Step by step

1. Confirm you are eligible before applying. You must live in a licensed residential care, assisted living, or intermediate or skilled nursing facility in Missouri. SNC does not help with rent or costs in your own home. Your monthly income must also be less than your facility's monthly cost.
2. Gather proof of your monthly income. This includes income from work, Social Security, and any other source.
3. Gather documentation showing the monthly cost of your facility.
4. Gather proof of Missouri residency.
5. Apply online through the Missouri Department of Social Services — Family Support Division's official application portal.
6. After submitting, watch for any follow-up requests from the Family Support Division. They may contact you if they need more information to process your application.
7. If approved, plan to renew your SNC benefits every year to keep receiving them.
