How to apply for Home Care Independence (hire your own caregiver) — Swain County

Who it's for
Home Care Independence lets Swain County residents age 60 and older hire a family member, friend or other person of their choice as a paid personal assistant. The assistant helps with personal care such as bathing and dressing, and with home tasks such as cooking, cleaning, laundry and shopping. A care advisor helps you plan and direct your own care, and a payroll service pays the assistant. Call Swain County Health Department at 828-488-3198 to ask about openings.
You must live at home, need help with at least one daily-living task, and be able to direct your own care (or have someone who does). The person who directs your care cannot also be your paid assistant. Funding is limited. If you have Medicaid, the CAP/DA waiver's consumer-directed option may fit better.
