Texas Medicaid for the Aged, Blind, or Disabled (the MEPD handbook categories at HHSC) is the SSI-related Medicaid pathway for low-income Texans who are age 65 or older, blind, or disabled and who do not require nursing home care. Eligibility is income-tested against the federal SSI Benefit Rate (FBR) — for 2026 the FBR is $994/month for an individual and $1,450/month for a couple — with a $2,000 countable-asset limit ($3,000 for a couple). Recipients who qualify for SSI cash benefits are automatically Medicaid-eligible in Texas; non-SSI applicants apply through HHSC. Medicaid covers physician visits, hospital care, prescription drugs, lab and X-ray, family planning, vision and hearing services, durable medical equipment, mental health care, and other services. Texas is one of the most restrictive Medicaid states for non-disabled adults. The Aged/Blind/Disabled pathway is the primary route for low-income seniors to access full Medicaid in Texas. The SSI-FBR income gate ($994/month single 2026) is stricter than many states use; households between the SSI FBR and 100% FPL may still qualify for the Medicaid Savings Programs (federal QMB/SLMB/QI) that pay Medicare premiums and cost-sharing without providing full Medicaid. For Texans needing in-home personal care or assistance with activities of daily living, STAR+PLUS HCBS (tx.star_plus) is the long-term-services pathway — distinct from regular ABD Medicaid. Medicaid recipients in Texas are typically enrolled in STAR+PLUS managed care if they are aged 21+ with disabilities or aged 65+. Married couples — long-term-care note: Texas applies spousal protections to Medicaid long-term care, including STAR+PLUS home- and community-based services and nursing-facility care. When one spouse needs long-term care and the other stays home, federal spousal-impoverishment rules protect the spouse who stays home (the community spouse). The single-applicant income and asset limits above are not applied to the community spouse's protected share — in 2026 that spouse can keep up to $162,660 in countable assets, plus a minimum monthly income allowance, while the spouse who needs care still qualifies.
