How to apply for Hospital financial assistance (Central Valley Medical Center) — Juab in Utah
Central Valley Medical Center
Who it's for
Central Valley Medical Center in Nephi has a Financial Assistance Program for patients who cannot pay for emergency or medically necessary care at the hospital and its clinics. Eligibility depends on your income (using Federal Poverty Level guidelines), family size, insurance and other ways to pay. You will need to verify your income with pay stubs, tax returns and bank statements, and apply for other help such as Medicaid. You have 120 days to finish the application. If you qualify, you will not be charged more than the amount usually billed to insured patients. For a free copy of the policy and application, call the Patient Billing Department at 435-623-3000, or pick one up at Admissions, Patient Billing or the Emergency Department.
Charity care is income-based and set by the hospital's financial assistance policy; the hospital makes the final award decision.
