How to apply for Medicare Savings Program (Arizona) — QMB / SLMB / QI-1

Arizona Health Care Cost Containment System (AHCCCS)

Who it's for

Medicare Savings Programs help people with Medicare pay their Part B premium, and (at the QMB level) also deductibles, coinsurance, and copays. Arizona has three levels — QMB, SLMB, and QI-1 — with rising income limits. In 2026 the highest level (QI-1) reaches about $1,796/month for one person and $2,435/month for a couple. Arizona applies NO asset/resource test to these programs. Apply at Health-e-Arizona Plus.

The income limit shown is the highest level (QI-1, 135% FPL). QMB (100% FPL, full premium + cost-sharing help) and SLMB (120% FPL) cut off at lower income. Arizona applies NO resource/asset test to the Medicare Savings Programs (the AHCCCS chart shows N/A) — do not assume the federal asset limits apply here. QMB enrollees should not be billed for Medicare-covered services.

What to say when you call

"Hello, I'm calling to ask about Medicare Savings Program (Arizona) — QMB / SLMB / QI-1. I'd like to know whether I may qualify, what documents I need, and whether I can apply online, by mail, by phone, or in person. I understand I may need medicare card (proof of Part A), proof of income, proof of Arizona residency — is anything else required? What is the best next step?"

Tip: have a pen ready, ask for the person's name, and write down any case or reference number they give you.

Step by step

  1. Gather your required documents: your Medicare card (proof of Part A enrollment), proof of income, and proof of Arizona residency.
  2. Check your income against the three program levels — QMB, SLMB, and QI-1 — to see which may fit your situation. The highest level, QI-1, reaches about $1,796 per month for one person and $2,435 per month for a couple in 2026. QMB and SLMB cut off at lower income limits. Arizona does NOT have an asset or resource test for these programs, so savings and property are not counted.
  3. Choose how you want to apply: online through Health-e-Arizona Plus, or by phone at 1-800-654-8713.
  4. Complete the application by answering all questions about your Medicare coverage, income, and Arizona residency.
  5. Submit your application and keep a copy of your confirmation for your records.
  6. Watch your mail for a notice from AHCCCS about your eligibility determination. Processing times vary, so contact AHCCCS at 1-800-654-8713 if you have questions about your status.

What to expect: After you submit your application, AHCCCS will review your information and mail you a notice explaining which level of the Medicare Savings Program — QMB, SLMB, or QI-1 — you qualify for, or whether more information is needed. Processing times vary; contact AHCCCS at 1-800-654-8713 for an estimate. If approved at the QMB level, Medicare providers should not bill you for Medicare-covered services. Your benefit must be renewed every year, so watch for a renewal notice from AHCCCS and respond promptly to avoid a gap in coverage.
