How to apply for Florida Optional State Supplementation (OSS)
Florida Department of Children and Families
Who it's for
Optional State Supplementation is a monthly cash payment from the State of Florida that supplements an SSI-eligible senior's income so they can afford the cost of an Assisted Living Facility (ALF), Adult Family Care Home (AFCH), or Mental Health Residential Treatment Facility. OSS is for low-income seniors and disabled adults who need help with the activities of daily living but who do not require nursing-home-level care. The state pays the difference between the resident's countable income and a published 'provider rate plus personal needs allowance' total — for an individual in an ALF the monthly income eligibility standard is $1,045.40 and the base provider rate is $991.40, plus a $160 monthly Personal Needs Allowance the resident keeps for incidentals (Florida Administrative Code 65A-2.036, effective June 24, 2025). To qualify, you must meet SSI's categorical eligibility (age 65+, blind, or disabled with countable income and resources below the SSI limits), be assessed as needing the level of care the facility provides, and reside in a state-licensed facility that accepts OSS payments. Eligibility is determined by DCF; payments are made monthly.
OSS only pays for residents living in a state-licensed Assisted Living Facility, Adult Family Care Home, or Mental Health Residential Treatment Facility that accepts OSS — not for seniors living independently at home. The facility must hold a current Florida Agency for Health Care Administration (AHCA) license and must be willing to accept the OSS provider rate. Confirm both your eligibility and your facility's OSS participation with DCF and the facility's admissions office before counting on this benefit.

What to say when you call
"Hello, I'm calling to ask about Florida Optional State Supplementation (OSS). I'd like to know whether I may qualify, what documents I need, and whether I can apply online, by mail, by phone, or in person. I understand I may need form CF-ES 2620 (OSS Eligibility Worksheet) and DCF ACCESS application, proof of identity and Social Security number, proof of all monthly income (Social Security award letter, pension statements) — is anything else required? What is the best next step?"
Tip: have a pen ready, ask for the person's name, and write down any case or reference number they give you.
Step by step
- Confirm that your facility qualifies by checking with both the Florida Department of Children and Families (DCF) and the facility's admissions office. The facility must hold a current Florida Agency for Health Care Administration (AHCA) license and must accept the OSS provider rate. OSS does not cover seniors living independently at home.
- Gather all required documents: Form CF-ES 2620 (OSS Eligibility Worksheet); proof of identity and Social Security number; proof of all monthly income such as a Social Security award letter or pension statements; bank statements and documentation of countable resources; proof of admission to a state-licensed Assisted Living Facility, Adult Family Care Home, or Mental Health Residential Treatment Facility (AHCA-issued license); and a level-of-care assessment confirming your need for residential care.
- Apply online through the DCF ACCESS application portal or call DCF at 1-866-762-2237 to apply by phone or get help starting your application.
- Submit Form CF-ES 2620 (OSS Eligibility Worksheet) along with your DCF ACCESS application and all supporting documents to DCF.
- Respond promptly if DCF contacts you for additional information or to schedule an interview or assessment. Missing a request can delay or stop your application.
- After approval, confirm with the facility's admissions office that the facility will accept OSS payments on your behalf before finalizing your living arrangements.
- Mark your calendar for your annual renewal. DCF will require you to re-verify your eligibility each year to keep receiving OSS payments.
What to expect: After submitting your application, DCF will review your documents, verify your income and resources, and confirm your level-of-care assessment. Processing times vary, so ask DCF for an estimate when you apply. If approved, you will receive a monthly cash payment that covers the difference between your countable income and the published provider rate, while you keep a Personal Needs Allowance each month for personal incidentals. Because OSS eligibility is reviewed annually, watch for renewal notices from DCF and submit any requested documents on time to avoid a gap in benefits.
