How to apply for Medicare Savings Programs (QMB, SLMB, QI) in Louisiana

Louisiana Department of Health (Louisiana Medicaid)

Who it's for

Medicare Savings Programs help people with Medicare Part A pay their Medicare costs. QMB pays the Part A and Part B premiums, deductibles, and coinsurance. SLMB and QI pay the Part B premium. From March 1, 2026, the monthly countable income limits are: QMB $1,330 for one person or $1,804 for a couple; SLMB under $1,596 or $2,164; and QI under $1,796 or $2,435. In Louisiana there is no resource (asset) limit for QMB, SLMB, or QI. People in these programs also get Extra Help with Medicare drug plan costs. Apply online, by mail or fax, in person at a Medicaid office, or by phone at 1-888-342-6207.

Shown against the highest tier (QI). Limits are for countable income, after allowed deductions. You must have Medicare Part A. People who get SSI are usually enrolled in QMB without applying. Louisiana Medicaid makes the final decision.

Step by step

  1. Check that you have Medicare Part A. These programs are for people with Medicare Part A, and people who get SSI are usually enrolled in QMB without applying.
  2. Review the countable income limits to see if you may qualify. From March 1, 2026, the limits shown here are for the highest tier, QI, and countable income is figured after allowed deductions.
  3. Gather the required documents. You will need the Application for Medicare Savings Program (BHSF Form 1-MB) or the Application for Health Coverage (BHSF Form 1-A), proof of income, and your Medicare card showing Part A.
  4. Apply online or apply by phone at 1-888-342-6207.
  5. Complete the application with your income details and Medicare Part A information. Louisiana Medicaid makes the final decision.
  6. Watch for requests for more information and respond if asked.

What to expect: After you apply, Louisiana Medicaid will review your application and decide whether you qualify for QMB, SLMB, or QI. If approved, the program can help pay certain Medicare costs, and people in these programs also get Extra Help with Medicare drug plan costs. Processing times vary, so ask the agency for an estimate if needed. Renewal is annual, so be ready to renew each year.
