How to apply for Medicare Savings Programs (QMB/SLMB/QI) — New Jersey

New Jersey Department of Human Services, Division of Medical Assistance and Health Services — administers NJ MSP

Who it's for

NJ Medicare Savings Programs (QMB, SLMB, QI) pay Medicare Part B premiums and (for QMB) cost-sharing. NJ uses the standard federal income limits (2026, per nj.gov): QMB ≤100% FPL ($15,960/yr single, $21,640 couple), SLMB 100-120% FPL ($19,152 / $25,968), QI 120-135% FPL ($21,546 / $29,214). NJ DID NOT eliminate the resource test like NY and CT did in 2023 — asset limits still apply ($9,950 single / $14,910 couple, 2026). Apply at the County Welfare Agency or via NJ FamilyCare. Combine with PAAD/Senior Gold for full Rx + Medicare relief.

NJ MSP still applies the federal asset limits ($9,950 single / $14,910 couple in 2026). NJ did NOT join NY/CT in eliminating the resource test in 2023.

What to say when you call

"Hello, I'm calling to ask about Medicare Savings Programs (QMB/SLMB/QI) — New Jersey. I'd like to know whether I may qualify, what documents I need, and whether I can apply online, by mail, by phone, or in person. I understand I may need medicare card, proof of NJ residency, proof of all household income and resources — is anything else required? What is the best next step?"

Tip: have a pen ready, ask for the person's name, and write down any case or reference number they give you.

Step by step

  1. Confirm you are enrolled in Medicare and live in New Jersey. NJ Medicare Savings Programs (QMB, SLMB, and QI) are open to NJ residents who meet income and asset limits.
  2. Check your income and assets against the 2026 limits. Income limits are: QMB — up to $15,960/year (single) or $21,640/year (couple); SLMB — $19,152/$25,968; QI — $21,546/$29,214. Asset limits are $9,950 for a single person and $14,910 for a couple. Note: New Jersey still applies these federal asset limits — the state did not eliminate the resource test as some other states have done.
  3. Gather your required documents: your Medicare card, proof of New Jersey residency, and proof of all household income and resources.
  4. Apply online through NJ FamilyCare or call 1-800-701-0710 to apply by phone or to find your County Welfare Agency. You may also apply in person at your local County Welfare Agency.
  5. Complete all questions on the application honestly and completely. Submit your documents along with your application.
  6. Ask about combining NJ MSP with the PAAD or Senior Gold programs if you also need help with prescription drug costs.

What to expect: After submitting your application, the agency will review your income, assets, and documents to determine which program — QMB, SLMB, or QI — you qualify for. Processing times vary, so contact the agency or your County Welfare Agency for an estimate. If approved, your Medicare Part B premium will be covered, and QMB enrollees also receive help with Medicare cost-sharing. Your coverage must be renewed every year, so watch for a renewal notice and respond promptly to avoid any gap in benefits.
