How to apply for New York Medicare Savings Programs (QMB / SLMB / QI)
New York State Department of Health — Medicare Savings Programs administered through county Departments of Social Services (NYC HRA for the 5 boroughs); pays Medicare Part B premiums (and sometimes deductibles + coinsurance)
Who it's for
NY Medicare Savings Programs help Medicare beneficiaries pay Part B premiums and (for QMB) deductibles and coinsurance. NY income limits are HIGHER than the federal floor, and NY has just two tiers: QMB up to $1,856/month single / $2,509 couple (138% FPL plus the $20 disregard, 2026); QI up to $2,494/month single / $3,375 couple (186% FPL plus $20). NO RESOURCE TEST in NY (the federal asset test was removed in NY as of 2023). Apply through county DSS, NYC HRA (212-331-3126), or the NY State of Health marketplace.
NY has NO RESOURCE TEST for MSP (eliminated 2023) and income limits are well above the federal floor. Most Medicare beneficiaries below ~$29,900 single income (about $40,500 for a couple) should at least qualify for QI (Part B premium relief).

What to say when you call
"Hello, I'm calling to ask about New York Medicare Savings Programs (QMB / SLMB / QI). I'd like to know whether I may qualify, what documents I need, and whether I can apply online, by mail, by phone, or in person. I understand I may need medicare card, proof of all household income, proof of NY residency — is anything else required? What is the best next step?"
Tip: have a pen ready, ask for the person's name, and write down any case or reference number they give you.
Step by step
- Confirm you are enrolled in Medicare and live in New York State — these programs are open to Medicare beneficiaries who meet the income guidelines. Note that New York has NO asset or resource test, so savings and property are not counted.
- Check your income against New York's limits: for 2026, QMB covers up to $1,856/month for a single person or $2,509/month for a couple; QI covers up to $2,494/month for a single person or $3,375/month for a couple. New York's limits are higher than the federal floor, so do not assume you are ineligible.
- Gather your required documents: your Medicare card, proof of all household income, proof of New York residency, and a photo ID.
- Apply online or by phone. Call the statewide helpline at 800-541-2831 to apply by phone or get help finding your local county Department of Social Services. If you live in New York City (any of the 5 boroughs), contact NYC HRA directly at 212-331-3126. You may also apply through the NY State of Health marketplace.
- Submit your completed application along with all required documents to the agency handling your case — your county Department of Social Services or NYC HRA.
- Watch for a written notice from the agency with a decision on your application. Processing times vary; ask the agency for an estimate when you apply.
What to expect: After submitting, the agency will review your application and documents and send a written decision. If approved, the program will pay your Medicare Part B premium directly — and if you qualify for QMB, it can also cover Medicare deductibles and coinsurance. Processing times vary, so contact the agency if you have questions about your status. Recertification is required once a year to keep your benefits, so watch for a renewal notice and respond promptly to avoid any gap in coverage.
