How to apply for Medicare Savings Program (Tennessee) — QMB / SLMB / QI
Division of TennCare
Who it's for
Medicare Savings Programs help people with Medicare pay their Part B premium, and (at the QMB level) also deductibles, coinsurance, and copays. Tennessee has three levels — QMB, SLMB, and QI — with rising income limits. As currently published, the highest level (QI) reaches $1,761/month for one person and $2,380/month for a couple; the resource limit is $9,950 (one person) or $14,910 (couple). Apply through TennCare Connect.
The income limit shown is the highest level (QI). QMB (full premium + cost-sharing help) and SLMB cut off at lower income. The figures are as published on the tn.gov TennCare page, which as of September 2026 still shows the 2025 Federal Poverty Level; the 2026 federal QI level is $1,816/month for one person and $2,455 for a couple (medicare.gov), so if you're close to the limit, apply anyway. QMB enrollees should not be billed for Medicare-covered services.

What to say when you call
"Hello, I'm calling to ask about Medicare Savings Program (Tennessee) — QMB / SLMB / QI. I'd like to know whether I may qualify, what documents I need, and whether I can apply online, by mail, by phone, or in person. I understand I may need medicare card (proof of Part A), proof of income, proof of resources — is anything else required? What is the best next step?"
Tip: have a pen ready, ask for the person's name, and write down any case or reference number they give you.
Step by step
- Gather your required documents: your Medicare card (proof of Part A enrollment), proof of income, proof of resources (such as bank statements), and proof of Tennessee residency.
- Check which level — QMB, SLMB, or QI — may fit your income. QMB offers the most help, covering the Part B premium plus deductibles, coinsurance, and copays. SLMB and QI cover the Part B premium and cut off at lower income levels than QI. If your income is close to any limit, apply anyway — figures can vary by source and year.
- Choose how to apply: complete the application online through TennCare Connect, or call TennCare at 1-855-259-0701 to apply by phone.
- Submit your application along with all required documents. Make sure your name, Medicare information, and contact details are accurate before submitting.
- Keep a copy of everything you submit and note the date you applied. If you applied by phone, ask the representative for a reference number or confirmation.
- Watch for a decision letter from the Division of TennCare. Processing times vary — contact TennCare at 1-855-259-0701 for an estimate if needed.
- Renew your coverage each year when TennCare sends your renewal notice. Missing the renewal deadline could cause a gap in your benefits.
What to expect: After submitting, the Division of TennCare will review your application and mail you a decision letter. Processing times vary, so contact TennCare directly for an estimate. If approved at the QMB level, you should not be billed for Medicare-covered services — notify your provider if you receive a bill. Coverage must be renewed annually; TennCare will send a renewal notice when it is time. If your income is close to any of the published limits, it is still worth applying, as the figures in use may be updated.
