How to apply for Senior services (Davis County Health Department) — Davis in Utah
Davis County Health Department — Senior Services
Who it's for
The Davis County Health Department runs senior services for Davis County. Its home and community-based programs help eligible older adults stay safe and independent at home, with help such as housekeeping or chores, personal care, bill paying, grocery shopping, medical equipment, emergency alert systems and running errands. Programs include Alternatives in-home services, caregiver support, support groups, the Veterans Directed Care Program and Medicaid waivers. Davis County also runs Rides4Health, curb-to-curb rides to medical visits for Davis County residents age 60 and older who cannot drive. The ride is free, but a $5.00 donation per trip is suggested; call 801-525-5061 to schedule. To see if you qualify for home services, call a case worker at 801-525-5050 or send an Information Request Form online.
AAA services are free or donation-based, and some have waiting lists. Most Older Americans Act services start at age 60. Utah Medicaid eligibility is decided by the State of Utah.
