Services: Aging & Disability Resource Center (ADRC) options counseling; enrollment/assessment for the Medicaid PathWays for Aging waiver and the state CHOICE program; Meals on Wheels and congregate meals; caregiver support (NFCSP); transportation (TRAM); and home modifications.
