Who it's for

Choices for Independence is Kalamazoo County's care-management program for residents age 60 and over. A licensed social worker or registered nurse (your Care Consultant) builds a plan around what you want and arranges the help — dental, respite and food vouchers, in-home care from providers under contract with the Area Agency on Aging, coordination of family supports, homemaking and personal care — and stays with you through hospital stays and transitions. Funding comes from the Older Americans Act, state funds and the county Senior Millage. Call the Area Agency on Aging at 269-373-5173 to ask about enrolling.
