Who it's for
U.P. Food as Medicine gives Menominee County adults who are at risk for or living with a chronic health condition vouchers to buy fresh fruits and vegetables at participating Upper Peninsula farm markets. Each voucher is worth $5, and participants get $15 a week during the 20-week market season — about $300 a season, mailed monthly. A doctor or clinic in the program's partner network sends the referral; a call specialist then completes enrollment. Ask your health-care provider for a referral, or dial 2-1-1.
Enrollment is by referral only: a health-care provider in the program's partner network must send the referral, and participants must be at risk for or diagnosed with a chronic health condition and face economic barriers to food. Vouchers buy only fresh fruits, vegetables, herbs and mushrooms — not eggs, meat, honey, bread or prepared food. Enroll by July 31 to receive vouchers for that season; anyone enrolling after August 1 receives vouchers the following spring. Participating markets and referral clinics vary across the Upper Peninsula — dial 2-1-1 to check what is available locally. The program makes the final decision.
