How to apply for Tennessee Family Support Program
Tennessee Department of Disability and Aging — delivered through county Family Support agencies
Who it's for
Flexible state-funded help so people with severe disabilities can stay in their homes with their families. Funds can pay for respite care, day care, home and vehicle modifications, equipment, personal assistance, transportation, and other supports the family chooses. It serves adults with severe disabilities (including those living with family or on their own without other residential program support) and families with children with severe disabilities. There is a Family Support agency for every county in the state — contact the Department of Disability & Aging regional office to find yours and apply.
The disability must be severe (a substantial, long-term limitation in daily activities). People already receiving HCBS waiver services (supported living, Employment and Community First CHOICES, Katie Beckett, or CHOICES) are not eligible. Awards depend on available funding and local council priorities, and may be less than the $6,000 limit. The agency makes the final eligibility decision.
