How to apply for Tennessee Family Support Program

Who it's for

Flexible state-funded help so people with severe disabilities can stay in their homes with their families. Funds can pay for respite care, day care, home and vehicle modifications, equipment, personal assistance, transportation, and other supports the family chooses. It serves adults with severe disabilities (including those living with family or on their own without other residential program support) and families with children with severe disabilities. There is a Family Support agency for every county in the state — contact the Department of Disability & Aging regional office to find yours and apply.

2. Gather your required documents: proof of the severe disability and proof of Tennessee residency.
3. Check that you meet the key eligibility rules: the disability must be a substantial, long-term limitation in daily activities, and you must not already be receiving HCBS waiver services such as supported living, Employment and Community First CHOICES, Katie Beckett, or CHOICES.
4. Apply online through the Tennessee Family Support Program application.
