1. Check your eligibility. You must be a Tennessee resident who is 65 or older, blind, or disabled, with income at or below $994 per month (single) or $1,491 per month (couple) and resources at or below $2,000 (single) or $3,000 (couple). If you already receive SSI, you are enrolled in TennCare automatically — no application needed.
2. Gather your required documents. You will need: proof of age 65+ (or proof of blindness or disability); proof of Tennessee residency; proof of income and resources; your Social Security card; and your Medicare card if you are enrolled in Medicare.
3. Choose how to apply. You can apply online through TennCare Connect or by phone at 1-855-259-0701. Pick whichever method is easiest for you.
4. Complete your application. Provide accurate information about your household, income, resources, and living situation. Answer all questions as completely as possible.
5. Submit your documents. Upload, mail, or provide your supporting documents as directed during the application process. Make sure every required document is included to avoid delays.
6. Watch for a decision notice. TennCare will review your application and mail you a notice with their decision. Processing times vary, so contact TennCare at 1-855-259-0701 if you have questions about your status.

What to expect: After submitting, TennCare will review your application and verify your documents. You will receive a written notice explaining whether you are approved or denied and what benefits you will receive. If approved, coverage is renewed annually, so watch the mail each year for a renewal packet and return it on time to keep your benefits. If your situation changes — such as income, resources, or living arrangements — report those changes to TennCare promptly. Note that this program covers standard Medicaid services for aged, blind, and disabled Tennesseans; if you need nursing-home or in-home long-term care services, you may need to apply through TennCare CHOICES instead, which has a different income limit.
