How to apply for Utah Medicaid Aging Waiver: in-home care for people 65+ instead of a nursing home
Utah Division of Aging and Adult Services (DAAS), through local Area Agencies on Aging
Who it's for
The Aging Waiver is a Utah Medicaid home and community-based services waiver that helps adults 65 and older stay in their homes or other community settings instead of a nursing home. To qualify you must be 65 or older, need nursing facility level of care, and meet Medicaid financial eligibility rules. Services may include adult companion services, adult day health, case management, chore and homemaker services, emergency response systems, home changes for accessibility, home-delivered meals, medication reminders, personal attendant services, respite care, medical equipment, and non-medical transportation. The waiver serves a limited number of people, and you get only the services you are assessed as needing. Contact your local Area Agency on Aging to start, or call the Division of Aging and Adult Services at 801-538-3910.
You must need the level of care given in a nursing facility (decided by an assessment) and be financially eligible for Medicaid. The waiver serves a limited number of people, so there may be a wait. DAAS and DWS make the final decisions.
