How to apply for Alzheimer's Family and Caregiver Support Program (AFCSP) — Wisconsin

Wisconsin Department of Health Services, through county and Tribal administering agencies (reached through the local Aging and Disability Resource Center)

Who it's for

Wisconsin's Alzheimer's Family and Caregiver Support Program helps families care for a person with Alzheimer's disease or another dementia at home. The person receiving care must have a dementia diagnosis and live in a home or community setting, not a facility. The program can pay for in-home respite care, adult day care, short emergency respite stays in a facility, home chore and home safety services, emergency response and alarm systems, caregiver counseling, education and support groups, activity supplies, special clothing, and legal costs for setting up guardianship. By state law the program can spend up to $4,000 a year for each person with dementia, and families who can afford it pay part of the cost on a sliding scale. To apply, contact your local Aging and Disability Resource Center (ADRC) at 1-844-947-2372.

Needs a doctor's diagnosis of Alzheimer's disease or another dementia, and the person must live at home or in a community setting. Funding is limited and set county by county, so there may be a wait. Families who can pay part of the cost are asked to do so on a sliding scale. People enrolled in Family Care usually get these services through Family Care instead. Your ADRC connects you to the local program.
