How to apply for Dementia Care Specialist Program (ADRCs) — Wisconsin
Wisconsin Department of Health Services, through the local Aging and Disability Resource Centers (ADRCs) and Tribal aging and disability resource specialists
Who it's for
Wisconsin's dementia care specialists give free information and support to adults with dementia who live at home and to the people who care for them. A specialist can do a memory screening and refer you to a doctor if it shows memory loss. They also help with care planning now and for the future, connect caregivers to local support groups, and point you to memory cafés and other social activities. Specialists work at your local Aging and Disability Resource Center (ADRC) or Tribal aging and disability resource specialist. Call 1-844-WIS-ADRC (1-844-947-2372) or visit FindMyADRC.org to reach yours.
The service is free. It is for adults with dementia who live at home and for their caregivers. Each ADRC or Tribal partner has its own specialist, so call your local ADRC to be connected. A memory screening is not a diagnosis; a specialist may refer you to a doctor.
