How to apply for Community Transition Services — help moving out of a nursing home (Michigan Medicaid)
Michigan Department of Health and Human Services (MDHHS), delivered by Area Agencies on Aging, Centers for Independent Living and other transition agencies
Who it's for
Community Transition Services help Michigan Medicaid members who live in a nursing home move back to a home or apartment in the community. A transition agency, such as your Area Agency on Aging or a Center for Independent Living, helps you look for housing and plan the move. One-time help can pay for a housing or security deposit, furniture, window coverings, kitchen items and linens, pest removal and cleaning, and utility start-up fees. It does not pay ongoing rent. You must live in a nursing facility and have Medicaid, or have a pending Medicaid application. To find out if you qualify, call MDHHS at 1-833-686-7700.
Only for people who live in a nursing facility now and have Michigan Medicaid (or a pending application that is likely to be approved); this check cannot confirm either from your profile. Transition Services are one-time costs to set up a home and do not cover rent or board. MDHHS decides who qualifies.
People who qualify for one senior program often qualify for others. See everything you may qualify for — free, no SSN, about five minutes.
