How to apply for Medicaid long-term care services (in-home, community, and facility care) in Oregon

Oregon Department of Human Services (ODHS), Aging and People with Disabilities

Who it's for

Oregon's Medicaid program (OHP) pays for long-term care for people with limited income and resources who need help with daily activities. Services can be given in your home, in the community, or in a care facility such as adult foster care, assisted living or a nursing facility. In-home help can include bathing, dressing, mobility, medications, and health-related tasks. For long-term care, income must be within 300% of the SSI standard ($2,982 a month in 2026), and resources must be $2,000 or less for one person. Rules protect a spouse who stays at home. People under 65 must have a disability decided by Social Security or the state. To apply, call the ADRC at 1-855-673-2372, apply online at ONE.Oregon.gov, or make an appointment at a local office.

You must also need help with daily activities at a level set by an ODHS assessment (service priority levels). The income limit applies to the applicant's own income; income over the limit may be handled with a qualifying trust. Part of your income may have to go toward the cost of care. The home's equity value is reviewed, and estate recovery may apply. ODHS makes the final decision.

What to say when you call

"Hello, I'm calling to ask about Medicaid long-term care services (in-home, community, and facility care). I'd like to know whether I may qualify, what documents I need, and whether I can apply online, by mail, by phone, or in person. I understand I may need medicaid (OHP) application, proof of income and resources, disability decision from Social Security (if under 65) — is anything else required? What is the best next step?"

Tip: have a pen ready, ask for the person's name, and write down any case or reference number they give you.

Step by step

  1. Gather your Medicaid (OHP) application, proof of income and resources, and a disability decision from Social Security if the applicant is under 65.
  2. Make sure the applicant needs help with daily activities at the level required by an ODHS assessment.
  3. Apply online or call 1-855-673-2372 to apply by phone.
  4. Give complete and accurate details about income, resources, and the kind of daily help needed.
  5. Respond if ODHS asks for more information or an assessment to decide the level of care needed.
  6. Watch for the final decision from ODHS.

What to expect: After you apply, ODHS reviews the application, checks income and resources, and decides if the applicant meets the care level requirement. ODHS may review home equity value, and part of the applicant’s income may need to go toward the cost of care. If income is over the limit, it may still be handled with a qualifying trust. Estate recovery may apply. Renew coverage each year.
