How to apply for Medicaid Purchase Plan (MAPP) — Medicaid for Working People with Disabilities (Wisconsin)

Wisconsin Department of Health Services (ForwardHealth), through the local income maintenance or Tribal agency

Who it's for

The Medicaid Purchase Plan (MAPP) gives full Wisconsin Medicaid coverage to adults with disabilities who work or want to work. It lets you earn more and keep more savings than other Medicaid programs for people with disabilities. You must be 18 or older, be found disabled by the state Disability Determination Bureau, have family income at or below 250% of the federal poverty level for your family size, and have $15,000 or less in countable assets (your home, one car and ABLE accounts do not count). You must do some paid or in-kind work at least once a month, or join a job-search counseling program. If your own monthly income is above the poverty level, you pay a monthly premium. Once enrolled, you can save your earnings in an Independence Account that does not count toward the asset limit. Apply online at ACCESS, by mail, or through your local agency. Call Member Services at 1-800-362-3002 with questions.

You must meet the MAPP work requirement (some paid or in-kind work at least once a month, or enrollment in the HEC job-search program); this check does not ask about work. The 250% income limit grows with family size; the screen here uses the one-person figure. If your own gross monthly income is above 100% of the federal poverty level you must pay a monthly premium, due by the 10th of each month. The state decides disability and eligibility.
