How to apply for Hospital Financial Assistance (Carnegie Tri-County Municipal Hospital) — Caddo in Oklahoma
Carnegie Tri-County Municipal Hospital
Who it's for
Carnegie Tri-County Municipal Hospital and Carnegie Medical Clinic say no one will be denied care because they cannot pay. They offer free or discounted care on a sliding fee scale based on family size and yearly income, using the current Federal Poverty Guidelines. People who qualify pay no more than a small fee of up to $5 and never more than insured patients are generally billed. Patients on Medicaid may be approved without a full application. Outside services, such as reference lab tests, drugs and a consulting radiologist's reading, are not covered. Fill out the Financial Assistance Application and attach proof of identity, income and insurance. You can apply up to 240 days after your first bill. Approval covers care back one year and six months ahead. Call the hospital at 580-654-1050.
Charity care is income-based and set by the hospital's financial assistance policy; the hospital makes the final award decision.
