How to apply for SoonerCare State Plan Personal Care (SPPC) in-home help

Who it's for
State Plan Personal Care is a SoonerCare (Medicaid) service that helps people of any age with daily living activities at home. A personal care attendant helps with things like bathing, grooming, getting in and out of bed, toileting, meals, eating, laundry, shopping and errands, and light housekeeping. Skilled nursing services include an assessment of need and building and overseeing a care plan. To qualify, you must meet SoonerCare's income and resource standards and meet medical level of care criteria. You can apply online, or call the Medicaid Services Unit at 1-800-435-4711 to set up a phone application.
You must qualify for SoonerCare and need help with daily activities, confirmed by a medical eligibility assessment. The amount of help depends on your care plan. OKDHS and OHCA make the final decision.
