Who it's for

The ADvantage Waiver is a SoonerCare (Medicaid) home and community-based waiver. It gives eligible adults an alternative to moving into a nursing home, so they can stay independent and close to family and friends. Services include case management, personal care, skilled nursing, home-delivered meals, medical equipment and supplies, prescription medications, home modifications, assisted living, adult day health, remote supports, and assistive technology. To qualify, you must be 65 or older, or an adult age 19 to 64 with a physical or other disability. You must also need the level of care given in a nursing home and qualify for SoonerCare financially. For waiver services in 2026, the applicant's own countable monthly income can be up to $2,982 (300 percent of the Federal Benefit Rate), and countable resources are generally limited to $2,000 for one person. You must need nursing-home-level care, confirmed by a medical eligibility assessment. Adults 19 to 64 qualify through a physical or other disability, not an intellectual disability or a cognitive impairment related to a developmental disability. The income limit counts only the applicant's own income, and a spouse who stays at home can keep more savings under special rules. Limits change each year. OKDHS and OHCA make the final decision.
