How to apply for Qualified Medicare Beneficiary (QMB)

Centers for Medicare & Medicaid Services / state Medicaid agencies

Who it's for

QMB pays your Medicare Part A and Part B premiums, deductibles, coinsurance, and copayments. If you qualify, you should not be billed for any Medicare-covered services. It's the most generous of the four Medicare Savings Programs and is widely under-enrolled — the federal government estimates millions of eligible Americans are not enrolled.

People who qualify for one senior program often qualify for others. See everything you may qualify for — free, no SSN, about five minutes.

What to say when you call

"Hello, I'm calling to ask about Qualified Medicare Beneficiary (QMB). I'd like to know whether I may qualify, what documents I need, and whether I can apply online, by mail, by phone, or in person. I understand I may need proof of identity (driver's license or state ID), proof of state residency, social Security card — is anything else required? What is the best next step?"

Tip: have a pen ready, ask for the person's name, and write down any case or reference number they give you.

Step by step

  1. Gather your required documents: a driver's license or state ID, proof of state residency, your Social Security card, your Medicare card, last month's bank statements, and proof of income (such as a Social Security award letter, pay stubs, or pension statements).
  2. Locate your state's application method. Every state listed offers an online application. Kentucky residents may also apply by phone at (855) 459-6328. Residents of all other listed states may also apply by phone at 1-800-MEDICARE (1-800-633-4227).
  3. Apply online through your state Medicaid agency's website — the official apply link is listed on this page — or call the phone number for your state to apply over the phone.
  4. Answer all questions on the application completely and accurately. You will be asked about your income, assets, and Medicare coverage.
  5. Submit your application along with copies of your required documents as directed by your state's application process.
  6. Keep a copy of your completed application and any confirmation number or receipt you receive for your records.
  7. Watch your mail for a decision letter from your state Medicaid agency. Processing times vary, so contact your state agency directly if you want an estimate of how long a decision may take.

What to expect: After submitting your application, your state Medicaid agency will review your information and mail you a decision letter. If approved for QMB, Medicare providers are not allowed to bill you for Medicare-covered premiums, deductibles, coinsurance, or copayments — so keep your approval notice handy. QMB enrollment must be renewed annually, so watch your mail each year for a renewal notice and respond promptly to avoid any gap in coverage.
