How to apply for Nevada Medicaid home care waivers (Frail Elderly and Physical Disabilities waiver)
Nevada Health Authority (Nevada Medicaid) and the Aging and Disability Services Division (ADSD); financial eligibility by the Division of Social Services (DSS)
Who it's for
Nevada Medicaid's home and community-based waivers help people who need nursing-home-level care get that care at home or in assisted living instead. The Waiver for the Frail Elderly (FE) serves people 65 or older. The Waiver for Persons with Physical Disabilities (PD) serves people of all ages with a documented physical disability. Services can include case management, homemaker and chore services, respite for caregivers, a personal emergency response system (PERS), home-delivered meals, and assisted living or augmented personal care. Nevada also covers nursing facility residents with gross monthly income up to 300% of the SSI payment level. Savings and other countable resources must be $2,000 or less for one person or $3,000 for a couple. To request services, contact your regional ADSD office.
You must need the level of care given in a nursing home and be at risk of moving into one without these services. There is also an income limit tied to the SSI payment level that changes each January. Waiver slots can be limited, so there may be a wait. Nevada Medicaid, ADSD, and DSS make the final decision.
