How to apply for ADvantage Waiver (SoonerCare home and community-based care for older adults and adults with disabilities) in Oklahoma

Oklahoma Human Services (OKDHS) Community Living, Aging and Protective Services, with the Oklahoma Health Care Authority (OHCA)

Who it's for

The ADvantage Waiver is a SoonerCare (Medicaid) home and community-based waiver. It gives eligible adults an alternative to moving into a nursing home, so they can stay independent and close to family and friends. Services include case management, personal care, skilled nursing, home-delivered meals, medical equipment and supplies, prescription medications, home modifications, assisted living, adult day health, remote supports, and assistive technology. To qualify, you must be 65 or older, or an adult age 19 to 64 with a physical or other disability. You must also need the level of care given in a nursing home and qualify for SoonerCare financially. For waiver services in 2026, the applicant's own countable monthly income can be up to $2,982 (300 percent of the Federal Benefit Rate), and countable resources are generally limited to $2,000 for one person. You can apply online, or call the Medicaid Services Unit at 1-800-435-4711 to set up a phone application.

You must need nursing-home-level care, confirmed by a medical eligibility assessment. Adults 19 to 64 qualify through a physical or other disability, not an intellectual disability or a cognitive impairment related to a developmental disability. The income limit counts only the applicant's own income, and a spouse who stays at home can keep more savings under special rules. Limits change each year. OKDHS and OHCA make the final decision.

What to say when you call

"Hello, I'm calling to ask about ADvantage Waiver (SoonerCare home and community-based care for older adults and adults with disabilities). I'd like to know whether I may qualify, what documents I need, and whether I can apply online, by mail, by phone, or in person. I understand I may need names of the home care and case management providers you would like for your county (from the OKDHS Provider Search tool), call the Medicaid Services Unit to ask what proof of income and savings to have ready — is anything else required? What is the best next step?"

Tip: have a pen ready, ask for the person's name, and write down any case or reference number they give you.

Step by step

  1. Check that this program fits your situation. You must be age 65 or older, or age 19 to 64 with a physical or other disability. You must also need nursing-home-level care and meet SoonerCare financial rules.
  2. Choose how to apply. Apply online, or call the Medicaid Services Unit at 1-800-435-4711 to set up a phone application.
  3. Pick the home care and case management providers you want for your county. Have the provider names ready from the OKDHS Provider Search tool.
  4. Call the Medicaid Services Unit to ask what proof of income and savings to have ready. Gather the documents they tell you to provide.
  5. Complete the application and give your chosen provider names and any requested financial documents.
  6. Take part in the medical eligibility assessment if asked. This assessment is used to confirm that you need nursing-home-level care.

What to expect: After you apply, OKDHS and OHCA review your information and make the final decision. They may contact you for more information or for a medical eligibility assessment. Processing times vary, so ask the agency for an estimate. If approved, plan to renew every year. Limits can change each year, and special rules may let a spouse who stays at home keep more savings.
