C. L. Brumback Primary Care Clinics + District Cares Specialty Benefits (Health Care District of Palm Beach County)
County · FloridaThe Health Care District of Palm Beach County (HCD) is a public special taxing district created by special act of the Florida Legislature in 1988 and funded by countywide property tax — i.e., every Palm Beach County property owner already pays into it. HCD runs an integrated public health system that includes the C. L. Brumback Primary Care Clinics (a federally qualified health center serving uninsured and underinsured Palm Beach residents), the District Cares Specialty Benefits program (which provides no-cost specialty care to eligible uninsured Brumback patients), Lakeside Medical Center in Belle Glade (the western county public hospital), the Trauma Agency, and pharmacy services. Adults at any income level can receive care at a Brumback Clinic — the clinics treat patients with or without insurance, regardless of ability to pay. For low-income uninsured patients, the sliding-fee scale based on household size and income reduces the visit fee to as little as a nominal co-pay. Community resource specialists at each clinic enroll qualifying patients into District Cares Specialty Benefits (free specialist visits, diagnostics, and outpatient procedures for eligible uninsured patients), Medicaid, the Health Insurance Marketplace, or other coverage. County · FloridaPalm Beach County's Older Americans Act Title III nutrition services are delivered through two paired lead agencies, divided geographically by Hypoluxo Road: the County Division of Senior and Veteran Services (DSVS) covers the area north of Hypoluxo Road, and the Mae Volen Center, Inc. (a nonprofit lead agency) covers the area south of Hypoluxo Road. Both deliver the same menu of services — congregate meals at neighborhood senior centers, home-delivered (Meals on Wheels) for seniors who cannot prepare their own meals, shop for food, and have no one to do that for them. Home-delivered meals come as a refrigerated or frozen tray plus milk, bread, and fruit, packaged separately and delivered on a regular schedule. Eligibility is age 60+ with no income or asset test, although for home-delivered the senior must meet the functional standard (inability to prepare meals or shop without help). As of January 18, 2026, Connection runs as two paired programs following an April 2025 Palm Beach BCC restructuring: Connection ADA provides door-to-door rides within the ¾-mile ADA service buffer around Palm Tran's fixed-route network (the federally mandated minimum); Connection Plus serves eligible customers whose trip origin or destination falls outside that ¾-mile area — a county-funded extension beyond ADA requirements. Both require an in-person eligibility determination through Palm Tran's existing certification procedure (a disability that functionally prevents fixed-route use; supporting medical documentation; in-person assessment). The standard fare is $4.00 per one-way trip; riders who qualify for a fare subsidy pay $3.50. No advance application or specialty card is required: proof of age at boarding is enough. For frequent riders, an All-Day Pass costs $3.50 and a 31-Day Pass costs $55, both at the reduced-rate price. Palm Beach EHEAP (Emergency Home Energy Assistance for Seniors)
County · FloridaThe Palm Beach Emergency Home Energy Assistance Program (EHEAP) is a senior-specific utility crisis benefit administered by the County Division of Senior and Veteran Services for households with at least one member age 60 or older. It is distinct from the general-purpose LIHEAP run by Palm Beach Community Action — EHEAP is sized smaller (up to $400 per season) but is delivered through the senior-service network so seniors are not competing with the broader low-income pool for funds. Households may receive one EHEAP benefit per heating or cooling season: up to $400 between April 1 and September 30 for summer cooling, and up to $400 between October 1 and March 31 for winter heating. The household must have been residing in the Palm Beach EHEAP service area at the time the energy bill was incurred and must present a final fuel bill showing an energy crisis (shut-off notice or similar). Households living in government-subsidized housing where utilities are included in rent are not eligible. Apply at the DSVS Administration office (810 Datura Street, West Palm Beach, 561-355-4746) or at one of three regional senior centers: Mid County Senior Center (Lake Worth, 561-357-7100), North County Senior Center (Palm Beach Gardens, 561-694-5435), or West County Senior Center (Belle Glade, 561-996-4808). Auto-qualification is granted if you already receive SNAP (Food Stamps), have applied for the Weatherization Assistance Program and been deemed eligible, or applied for Community Services Block Grant and been deemed eligible. The benefit is capped at $400 per season — total annual exposure is $800 if you draw both the cooling and heating benefits. Households in government-subsidized housing with utilities included in rent are NOT eligible.
